I, hereby, permit you to debit my savings account no. 123456 with the annual rental fees of Rs.2000 and the initiation charges of Rs. 5000 for the same.
Please find attached all the required self-attested documents for the procedure. I also request you to please arrange for debiting the annual rental fee for locker from my savings account, the number of which has been mentioned above.
